The welding certifications were checked and double-checked.
The inspectors looked at the seams and saw beautiful, clean lines. Everything on paper said these ships were the pinnacle of modern engineering.

We need to start asking better questions. Instead of asking “Is this signed?” we should be asking “Where is the room where the work happens, and why aren’t we in it?” We need to be willing to embrace the friction of the physical world.
